The CAREMED Project, co-funded by the European Union under the Interreg NEXT MED Programme, has launched the implementation of the Active Patient Care Model in Italy, one of the two innovative care models at the heart of the project. The model will be adapted and piloted across CAREMED partner countries to strengthen community-based approaches for the prevention and management of chronic diseases.
A first online training session was held on 27 May 2026, bringing together 56 Italian healthcare professionals and marking the start of a training pathway that will continue with an in-person programme from 3 to 6 June, involving professionals from the Primary Healthcare Centres (Case della Comunità) of Florence and Lucca.
The session was led by the Gerència d’Atenció Primària de Mallorca (GAPM), CAREMED partner, which presented the methodology and implementation pathway of the Active Patient Care Model. The model follows a cascading approach: healthcare professionals are trained first and then train expert patients, who will facilitate peer-support groups and focus group activities for people living with the same chronic conditions, promoting self-management and active participation in community care.
A valuable contribution came from two Active Patients involved in the programme in the Balearic Islands, who shared their experiences and the positive impact of peer-to-peer support in their communities.
This milestone marks an important step for the Italian implementation of the model and offers healthcare professionals the opportunity to learn from an established experience in patient engagement and chronic disease management.
Through the transfer and adaptation of innovative healthcare models across the Mediterranean, CAREMED continues to promote more resilient, inclusive, and patient-centred health systems, where patients become active partners in shaping healthier communities.
